Scars are a natural part of healing, but in some people the body produces too much collagen, resulting in raised, thickened scar tissue. Hypertrophic scars are raised and firm, often red or pink, but stay within the boundary of the original wound and may slowly improve over 1–2 years even without treatment. Keloid scars are different — they grow beyond the original wound boundary and can continue expanding over time. They are more common in clients with medium to darker skin tones, and typically appear on the chest, back, shoulders, jawline and earlobes (often after piercing). Confusing the two matters, because they are treated differently and respond differently.
Keloids are one of the most challenging scar types to treat, anywhere in the world. They have an active growth phase and can recur if not managed correctly. Surgical removal alone almost always causes a keloid to return, often larger than before, and the immune response that drives keloid formation may continue even after treatment — aggressive treatment such as laser or excision on keloid-prone skin can even trigger new keloid formation at the treatment site. Corticosteroid (triamcinolone) injection directly into the scar tissue is the primary treatment option discussed at DIVA Skin Clinic Poipet for keloid and hypertrophic scars — it works by reducing inflammation and decreasing collagen production within the scar, causing it to soften and flatten over time. For red or vascular-looking scars, a vascular/pulsed-dye type laser may be discussed alongside injection to help reduce redness and improve pliability. Softening and flattening of the scar typically begins within 2–4 weeks of the first injection. Multiple injections are usually needed — generally spaced 4–6 weeks apart — with around 3–6 injections discussed for significant flattening in most cases. Clients with a strong tendency to form keloids may need ongoing maintenance injections even after initial improvement. The exact number and interval is assessed individually based on scar size and how it responds.
Injection into firm scar tissue can cause moderate discomfort during the procedure itself, and topical numbing may be used for larger keloids. This is not an ablative, resurfacing-style downtime — but clients are asked to avoid swimming pools, saunas or excessive sweating for about a week after injection, and to avoid picking, scratching or otherwise traumatizing the treated area while it settles.
Keep the treated area clean and moisturized, and apply SPF 50+ over healed scars, since UV exposure can darken scar pigmentation. Do not pick, scratch or otherwise traumatize the area, and avoid swimming pools, saunas or excessive sweating for about a week after injection. Report any unusual reaction — increased pain, swelling or redness — to DIVA Skin Clinic Poipet immediately. If you are prone to keloids, tell DIVA before any new procedure, including piercings or minor surgery elsewhere, since new skin trauma can trigger new keloid formation.
Keloids are genuinely difficult to treat, and DIVA Skin Clinic Poipet will not promise complete elimination — the realistic goal is flattening, softening, reducing discomfort and controlling further growth, not guaranteed permanent removal. Keloids can recur after treatment, especially in clients who are highly prone to them, and aggressive treatment such as laser or surgical excision on keloid-prone skin can in some cases trigger new keloid formation at the treatment site, which is why DIVA favours a careful, staged approach over aggressive single-step removal. Triamcinolone injection does not remove the dark pigmentation of a scar — it reduces volume and inflammation. Very large keloids may need combination treatment. Results vary significantly by individual scar type, size, genetic tendency, number of sessions and aftercare — this page does not promise a specific, complete or permanent result, and this is not a guaranteed cure.
